“Identity verification failed” means the details on your application did not match the Department of Home Affairs population register — usually a name spelt differently, an outdated surname, or a fraud-prevention flag because your ID number was used by someone else.

Is this decline commonly wrong?

Half and half. Genuine mismatches (maiden vs married surname, spelling differences, a second name used as a first name) are common and fixable. But this reason also fires when someone else applied with your ID number — which you must take seriously as identity fraud.

Evidence that overturns it

  • Your green barcoded ID book or smart ID card.
  • If your name changed: the Home Affairs document reflecting the change.
  • Completion of SASSA's online identity verification (see below) — this is the actual fix.

How to appeal

Lodge your appeal on the official appeals portal at srd.dsd.gov.za within 90 days of the outcome — and separately for each month that was declined.
